Prima Service Request Form

Terms of Service - All commercial service work is billed out on a time and materials basis. Our rates are $175/Hour for Labor and $230/Hour for Overtime Labor. There is a 1hr minimum and work is billed out in 30 minute increments after that. If account becomes in arrears after 30 days, we may charge your credit card to keep your account active. Late charge/finance fee is 2.5% per month.
